Breast augmentation (mammoplasty) enlarges and shapes the female breast—either with Silicone implants or the body's own Autologous fat (Lipofilling). The procedure takes about 1–2 hours and takes place in our certified operating rooms in Oberursel. The goal is to achieve natural proportions that suit your body.
Which procedure is appropriate depends on the initial findings: the existing breast shape, the thickness of the soft tissue envelope, and how much volume is desired. Implants allow for more precise planning of the size. With lipofilling, fat is harvested from another area and injected into the breast; this requires sufficient fat reserves, and the volume increase per session is limited because only a portion of the transferred tissue takes hold. This procedure differs from a breast lift in its goal: a breast lift focuses on shape and position with sufficient volume, whereas this procedure focuses on the volume itself. Breast Augmentation with Implants:
After administering general anesthesia, we make an incision—depending on the consultation, in the inframammary fold (the most common method), in the armpit, or at the edge of the nipple. We create a pocket for the implant, either above the pectoral muscle (subglandular), below the pectoral muscle (submuscular), or using the modern dual-plane technique (partially under the muscle). The implant is positioned, and the incision is closed with fine sutures. Once healed, the scars are barely visible.
Breast Augmentation with Autologous Fat:
Under general anesthesia or twilight sedation, we gently harvest fat tissue from suitable areas of the body (abdomen, hips, thighs). The fat is processed and precisely injected into the breast using fine cannulas. We deposit many small fat deposits in different layers to ensure that the fat cells receive an optimal blood supply and can take. About 70% of the injected fat is permanently retained.

General anesthesia: The standard procedure for breast augmentation with implants
Twilight Sleep: Often sufficient for autologous fat treatments

The First 48 Hours: Hospital Stay and Rest
After the surgery, you will stay at our clinic for 1 to 2 nights for observation (autologous fat transfer is often possible on an outpatient basis). A special support bra will be put on immediately after surgery to stabilize the breast. In the first few days, you may experience a feeling of tightness, mild pain, and swelling—this is completely normal. Keep your upper body slightly elevated and avoid sudden movements.
Days 3–7: Mobilization at Home
Starting on the third day, you may shower carefully. You should wear the support bra at all times. Short walks are important, but strenuous activities should be avoided. Do not lift heavy objects (over 5 kg) or make overhead movements. The swelling is at its worst but will subside each day.
Week 2: Suture Removal
The stitches are removed after 7 to 10 days (unless they are dissolvable). Most patients feel increasingly comfortable and can resume light office work. The support bra should continue to be worn day and night.
Weeks 3–6: Return to Normalcy
After 2 to 3 weeks, you will be largely mobile again. Your breasts will still feel firm and sit slightly higher – they will „drop“ into their final position over the coming weeks. The support bra should be worn for a total of 6 weeks. After 4 weeks, you can wear a regular bra again.
Sports, Sun, and Sauna: Take a 6–8-week break
Give your body at least 6 weeks off from exercise that puts strain on the chest (jogging, upper-body strength training). Light walks are possible starting in week 2. You should avoid saunas and swimming pools for 6 weeks. Protect the scars from sunlight for 12 months by using sunscreen with an SPF of 50+ or higher.
The final result after 3–6 months
The full, final results of your breast augmentation will be visible after 3 to 6 months—by then, the implants will have settled into their final position, all swelling will have subsided, the scars will have faded, and you’ll be able to fully enjoy your natural, feminine cleavage.

Modern implants last 10 to 20 years or longer. They do not last a lifetime, but they do not need to be replaced routinely as long as no problems arise. Replacement is necessary only in cases of discomfort, capsular fibrosis, or implant damage.
Breast augmentation can affect your ability to breastfeed, especially if the incision is made through the nipple. When the incision is made in the inframammary fold, the milk ducts usually remain intact. However, there is no guarantee. If you haven’t finished planning your family yet, we should take this into account during the planning process.
Modern silicone implants filled with highly cohesive gel feel very natural and are virtually indistinguishable from natural breast tissue to the touch. Choosing the right size and implant placement is crucial. If the implants are too large or there is insufficient natural breast tissue, the implants may be palpable.
Round implants provide more fullness in the upper breast area (cleavage) and are the first choice for most patients. Anatomical (teardrop-shaped) implants mimic the natural shape of the breast but can rotate. We’ll provide you with personalized advice on which shape best suits your preferences and your body type.
The dual-plane technique (partially under the muscle) is now standard: The implant is placed under the muscle in the upper portion and over the muscle in the lower portion. This ensures natural contours and minimizes the risk of capsular contracture. For very athletic women, placement over the muscle may be more appropriate.
If you’d like to increase your cup size by 1–3 sizes and have sufficient fat reserves, both options are possible. Implants provide more volume and can be precisely planned. Autologous fat is 100% natural and leaves virtually no scars, but it requires sufficient body fat and yields moderate results. We’ll advise you based on your wishes and individual circumstances.
