The Bullhorn Lip Lift – named for the characteristic shape of the incision, which resembles a bull’s head – is a minor surgical procedure in which a crescent-shaped strip of skin is removed directly below the nose. Lifting the upper lip shortens the philtrum, reveals more of the white part of the lips, and lifts the entire lip area.

If the distance between the nose and the upper lip is too great
As we age, the philtrum—the area between the base of the nose and the edge of the upper lip—lengthens. At the same time, the upper lip loses projection and turns inward. The white part of the lip disappears, and the lip appears narrow and pulled downward. An ideal philtrum measures about 11 to 13 mm in women—if it is longer than that, a lip lift can significantly rejuvenate the face.

When is a medical evaluation advisable?
If you are prone to scarring (keloids or hypertrophic scars), we will discuss in advance whether the procedure is appropriate. Female smokers should consistently avoid nicotine for at least 4 weeks before and after the surgery, as it impairs wound healing. If you have an active cold sore, we will postpone the surgery and recommend antiviral prophylaxis.

Preparation:
The incision line is marked precisely—directly at the base of the nose, tailored to the individual shape of your nostrils. This marking is crucial for the final result and for ensuring the scar is inconspicuous. Local anesthesia is administered and takes effect within a few minutes.
The procedure:
A crescent-shaped strip of skin—usually 3 to 8 mm wide, depending on the desired shortening—is removed directly below the nose. The remaining skin is pulled upward and closed with very fine, self-dissolving sutures. The external skin suture is made with non-absorbable suture material and removed after 5 to 7 days. Thanks to the precise incision at the base of the nose, the scar is hidden in the natural skin fold.
Aesthetic fine-tuning:
After closing the wound, we assess the result while the patient is in a relaxed position with a smile. Minor adjustments are made immediately to ensure optimal symmetry.
Combination with other treatments:
If desired, the Bullhorn Lip Lift can be combined with a small amount of hyaluronic acid during the same session—to add volume while the lips are already lifted. A Corner Lip Lift can also be performed during the same session if needed.
Where does the treatment take place?
In our own certified treatment rooms at Valentine Plastik—on an outpatient basis, discreetly, and in a trusting atmosphere.
Local anesthesia: This is standard for the Bullhorn Lip Lift—you’re awake but feel nothing. Many patients prefer this option because they can go home right away afterward.

The First 24–48 Hours: Rest and Cooling
Immediately after the procedure, the upper lip area will be swollen and slightly numb due to the anesthesia. Gently cool the nose and lip area with a soft cooling pad. Sleep with your upper body slightly elevated. Avoid vigorous facial movements, wide grins, and strenuous chewing for the first 24 hours.
Days 3–7: Swelling subsides and sutures are removed
The swelling subsides a little more each day. Small bruises around the nose are possible and normal. After 5 to 7 days, the external stitches will be removed—a quick, painless procedure. From that point on, you can gently cover the incision and apply light makeup.
Week 2: The results are becoming visible
Most of the swelling has subsided. You can now see the lifted result—your new lip line. The scar is still reddish at this stage, which is completely normal. You can resume light office work starting in week 2.
Weeks 3–6: Daily Life and Scar Care
Starting in week 3, you can return fully to your normal daily routine. Scar care begins: apply scar ointment or silicone gel daily, and gently massage the scar once it has fully healed. The scar will become less noticeable with each passing week.
Exercise, Sun, and Sauna: Take a 2–4-week break
You should avoid strenuous exercise for 2 to 3 weeks, as high blood pressure can interfere with healing. Avoid saunas and swimming pools for at least 3 weeks. Very important: Consistently protect the fresh scar from UV radiation with SPF 50+ sunscreen for at least 12 months—sun exposure significantly slows down the fading process.
Scar Care Starting in Weeks 2–3
Once the wound has completely healed, intensive scar care begins. Applying special scar ointments (e.g., Contractubex, Dermatix) or silicone gel daily, along with gentle scar massages, promotes healing. Professional scar treatments such as laser therapy or microneedling can further optimize the results—please ask us about these options.
The final result after 6–12 months
The final result—including a completely faded scar—becomes apparent after 6 to 12 months. The lifted lip position is permanent, and the scar fades to a barely visible, light-colored line right at the base of the nose.
